PPO is Maintained by Treasury (Districts) and Pension Disbursement Cell (Kolkata)

There are two options available.

Option 1: Submission of LC Digitally using the Aadhar based Jeevan Praman Application using a Biometric Thumb Scanning Device.

Prerequisite for using Jeevan Praman Option:

The Aadhar Number needs to be linked with the PPO in the respective Pension Disbursing Treasury by submitting a copy of the self attested Aadhar Card to the Treasury, mentioning the PPO No/ID, if not already linked or updated.

Note:

Submission of LC digitally using the Jeevan Praman Application may be done from / by:

• Home by downloading the application and using any Biometric Thumb Scanning Device.
• Visiting the Pension Receiving Bank Branch.
• Visiting the nearest Treasury.
• Visiting any Common Service Kiosks (CSK) providing this facility.
• Visiting / Contacting the nearest India Post Payment Bank under the Department of Post.

Option 2: Downloading the LC Form (Life Certificate Form.pdf) and physically submitting the same after being duly Certified by any authorized person.

List of Authorized Persons

After downloading the LC Form, physically submit it to:

• The Pension Disbursing Treasury in the respective Drop Box.
• The Pension Receiving Bank in their Counter / Drop Box.

Note:

The pensioner need not visit the Bank / Treasury for the submission. The physical submission may be made through any messenger/post.
